Family Nurse Practitioner

 Duties: Complete yearly HRA Assessments for United HealthCare. Provide
 Physical Examination, Educate on conditions and medications. Refer to Case management when
 needed. The HRA promotes health awareness by reviewing participants' personal lifestyle practices and
 revealing health issues that could be impacted by personal choices. Managing primarily the geriatric
 population. Advising clients with Depression, CAD, Anxiety, Hypertension, Hyperlipidemia, CKD, COPD,
 Schizophrenia, LBP, OA, Bipolar, Diabetes, PVD, Seizures, Obesity, etc. close gaps in care

 Opioid Addiction Clinic. Suboxone waiver. Prescribe, manage, and treat patients with opioid addiction
 with suboxone. Patients have monthly and as needed visits along with seeing LPC at facility.

 Duties: Complete yearly HRA Assessments for Aetna and Wellcare. Provide
Physical Examination, Educate on conditions and medications. Refer to Case management when
 needed. The HRA promotes health awareness by reviewing participants' personal lifestyle practices and

 population. Advising clients with Depression, CAD, Anxiety, Hypertension, Hyperlipidemia, CKD, COPD,
 Schizophrenia, LBP, OA, Bipolar, Diabetes, PVD, Seizures, Obesity, etc. close gaps in care.

 Psychogenic management of psychotropic Medications in LTC facilities and rehab. Dx and treat mental
 illness with psychiatrist and psychologist. Use diagnostic testing and evaluation. Managing conditions
 such as Schizophrenia, Alzheimer’s, Depression, Insomnia, Anxiety, Bipolar, Obsessive Compulsive
 Disorder, and PTSD

 Primary Care, Opioid Addiction Treatment with Suboxone. Dx and treat acute and chronic conditions.
 Manage psychotropic medications in all age groups. Mange conditions such as Diabetes, Hypertension,
 Hyperlipidemia, Congestive Heart Failure, Coronary Artery Disease, Chronic Kidney Disease, HIV,
 Depression, Insomnia, Anxiety, Schizophrenia, Bipolar, Obesity, Epilepsy, and PTSD
