Director of Mental Health Department

•	Provide Psychiatry mental health evaluation, screening, treatment, and education of adults and family 
•	Prescribed appropriate psychotropic medications and manage effectiveness 
•	Work in collaboration and under the supervision of the psychiatrist to establish and provide excellent and safe mental health services  
•	Able to effectively diagnose mental health illnesses, manage and coordinate the mental health care of the clients    
•	Provide health education and counseling and make appropriate referrals
•	Maintain health records according to HIPPA guidelines
•	Direct and collaborate with providers for specific conditions/crisis that may require hospitalization under the Bake Act criteria or any change in psychiatric condition   



Psychiatric Mental Health Nurse Practitioner

•	Provide nurse case management to injured workers, assisting in restoring them to maximum medical improvement and prompt gainful employment through effective communication and utilization of appropriate resources to promote quality, cost-effective outcomes.
•	Educate patients and family regarding medication, illness management, special treatment, and self-care and establish and gain trust
•	Provide superior and culturally sensitive service to patients and family  
•	Provide medical case management service consistent with URAC standards and CMSA Standards of Practice and Quality Improvement (QI) Guidelines to patients/employees receiving benefits under an Insurance Line, including but not limited to Workers' Compensation, Group Health, Liability, and Disability.

Field Nurse Case Manager

•	Administer primary nursing care, working with the treatment team and clients to implement a care program and reinforce skills learned in various therapies.
•	Expertise in coordinating discharge planning and home care with doctors, nutritionists, physical therapists, occupational therapists, and social workers; strong on documentation and follow-through.
•	Specialize in tracheotomy care, wound care, and IV therapy.
•	Collaborate with staff and physicians to improve patient and staff education. 




Staff Nurse

•	Supervise the delivery of patient care, including managing and supervising licensed and non-licensed personnel in this 120-bed geriatric, long-term, multi-level, and skilled facility.
•	Responsible for personnel management and coordinating nursing services activities for the scheduled working shift in accordance with the established philosophy, objectives, and policies of the hospital to ensure safe, efficient, and therapeutic patient care.
•	Actively participate in all admission and discharge planning. 
•	Responsible for delivery of efficient, safe patient care using the nursing process hospital policies and procedures.

A follow-up visit with a provider involves checking the progress of the patient's treatment plan, assessing any new symptoms or concerns, and adjusting the treatment plan as needed to ensure optimal health outcomes.

Follow-up Visit w/ Medication Refills

An opportunity for potential patients to discuss their concerns or needs with a provider, or therapist, to determine if their services are a good fit and to receive initial advice or guidance. Many providers offer free or very affordable consultation services with potential patient leads. Providers who add Free consultation as a service receives 2-3 times more prospects.
