In my role, I am responsible for the close monitoring and treatment of 41 patients in a detoxification setting. I ensure that nightly chart audits are conducted and that all medications are administered according to established treatment protocols. Additionally, I assist with patient intake, which involves handling insurance authorizations, conducting medical assessments, and notifying healthcare providers. I also formulate nursing care plans that encompass the assessment, planning, implementation, and evaluation of treatment effectiveness, ensuring a comprehensive approach to patient care.

I support the behavioral management of patients during the detoxification process by employing verbal redirection and active listening techniques to offer reassurance and comfort. I administer medications such as methadone and suboxone in accordance with established treatment protocols, while carefully monitoring patients using CIWA and COWS assessments. Additionally, I provide comprehensive education to patients about the detoxification process, ensuring they are informed and supported throughout their recovery journey.

I remotely triaged, diagnosed, and treated patients through online chat and video platforms, providing guidance on pre- and post-operative instructions as well as medication administration. Based on assessments, I recommended appropriate dispositions to physicians. I monitored patient compliance and evaluated the effectiveness of home treatment instructions to optimize outcomes. I conduct physical examinations for all patients upon admission, including annual physical and health examinations, and review admissions-related laboratory results with them. I assess, diagnose, and prescribe various medications. All examinations I perform are consistent with applicable state rules and regulations. I issue patient medication, change orders, and verbal orders as needed. I am available to staff for emergency management of patient care. I provide reports as requested and attend weekly staff meetings and treatment team meetings with counselors, or document alternatives in the patient record. I complete all appropriate clinical documentation forms on the EMR platform. I provide monthly and annual reports for infection control and complete follow-up and investigation on medical incident reports.

PMHNP - Mental Health & Addiction

I conduct comprehensive patient assessments, including gathering medical histories, performing physical examinations, and evaluating psychosocial factors. I prescribe and manage medications, including medication-assisted treatment (MAT) for substance use disorders, in accordance with state and federal regulations. I develop and implement compassionate, individualized treatment plans in collaboration with patients and the multidisciplinary team, focusing on treating the whole person rather than just symptoms. I monitor patient progress and adjust treatment plans as needed to achieve optimal outcomes. I provide education and counseling to patients and their families regarding substance use, treatment options, and health maintenance. Additionally, I collaborate with physicians, therapists, social workers, and other healthcare professionals to ensure a coordinated approach to patient care.
