Patient Education: Educate patients and their families on their conditions, treatment plans, medication usage, and preventive health measures.
Follow-Up Care: Monitor patient progress, adjust treatments as necessary, and provide ongoing care management.

Assessment: Conduct comprehensive assessments of patients’ health status, including physical, emotional, and psychosocial aspects.
Care Planning: Develop individualized care plans based on patient assessments, collaborating with multidisciplinary teams to ensure a holistic approach.
Implementation: Administer medications, treatments, and procedures according to established protocols and patient care plans.
Monitoring: Continuously monitor patient progress, identifying changes in condition and adjusting care plans as needed.

An initial visit with our provider which will involve a medical history review, an assessment of your specific concerns, goals and health care needs, possible diagnostic tests to evaluate our patient's health, identify issues, and develop and discuss treatment options. At this consultation, we will discuss which weight loss package is right for you
The $50 consultation fee is applied to 1st month’s plan if the patient decides to move forward with the program
