My name is Jacqueline Horne, and I'm a Family Nurse Practitioner dedicated to providing compassionate and comprehensive healthcare. With over twenty years of experience in various settings, including medical/surgical, psychiatric, and correctional facilities, I have honed my skills in assessing and treating a wide range of health conditions.
I have worked as a primary and acute care provider, visiting high-risk patients in their homes to offer care and prevent hospital admissions. My background also includes providing critical care to COVID-19 patients, managing chronic conditions, and offering wound care and emergency services.
Communication is key in my practice. I work closely with doctors, patients, and healthcare teams to ensure everyone is on the same page. I believe in creating personalized care plans tailored to each patient's needs, helping them achieve the best possible outcomes.
