Documentation requests (accommodation forms/letters of medical necessity) require an established care relationship and ongoing evaluation. These services are not provided during initial or one-time telehealth visits.

This visit includes a comprehensive medical evaluation to determine eligibility for prescription weight loss treatment. Your provider will review your health history, discuss treatment options, and develop a personalized plan when clinically appropriate.

Medication costs are separate from the visit fee and are based on prescribed dose. There are no monthly membership fees.

An initial visit with a provider involves a medical history review, an assessment of your specific symptoms and concerns, and possible diagnostic tests to evaluate a patient's health, identify issues, and develop and discuss treatment options.

A follow-up visit with a provider involves checking the progress of the patient's treatment plan, assessing any new symptoms or concerns, and adjusting the treatment plan as needed to ensure optimal health outcomes.

A medication refill appointment with a provider typically involves a brief review of his/her existing patient's medical history, confirming the need for the medication refill, and issuing a new prescription. It may also involve evaluating any side effects or concerns related to the medication and ensuring that the treatment plan is still appropriate for the patient's needs.

This visit includes an evaluation of skin, scalp, or nail concerns such as rashes, psoriasis, fungal infections, dandruff, itching, skin discoloration, bug bites, lice, scabies, shingles, cellulitis, and other common dermatologic conditions, along with a personalized treatment plan.

Offers specialized care for skin, hair, and nails, treating a wide range of skin conditions, performing skin surgeries, and providing cosmetic enhancements.

This follow-up visit is for ongoing or previously evaluated skin, scalp, or nail conditions. The provider will assess response to treatment, address any new or persistent symptoms, and adjust the treatment plan as needed.

Evaluation and treatment of acute infections, including respiratory infections, UTIs, skin infections, gastrointestinal illness, and select sexually transmitted infections, with individualized treatment and follow-up guidance.

Focuses on the diagnosis, treatment, and management of infectious diseases; managing acute and chronic infections, providing vaccinations, and offering travel health advice.

This follow-up visit is for patients previously evaluated for an acute infection. The provider will review symptom progression, response to treatment, and any new or persistent concerns.

The treatment plan may be continued, adjusted, or completed based on clinical response. Patients with worsening symptoms or signs of a more serious infection may be referred for in-person evaluation.

During a weight loss consultation, I assess the patient's medical history, lifestyle, and weight loss goals. Based on this comprehensive evaluation, I develop a personalized weight management plan that may include dietary and exercise recommendations. If appropriate, I prescribe weight loss medication to support the patient’s efforts, ensuring regular follow-ups to monitor progress and adjust the plan as needed.
