•	Treating 15-30 patients per day 
•	Taking accurate and pertinent history
•	Devising individualized, evidence based Plans of Care 
•	Prudent charting and both verbal and written communication
•	Prescribing necessary medications, labs and interventions
•	Collaborate and follow progress of each patient, making changes to plans of care as needed
•	Maintains a professional demeanor, teaching patients and providing support.

Family Nurse Practitioner

•	Monitoring patients straight from the ER
•	Provided safe and effective services resulting in increased HCAHPS scores.
•	Monitor patients closely and giving detailed report to unit manager and incoming shift nurse

•	Care for a variety of patients requiring diverse, individualized care
•	Collaborate with therapists, social workers and specialists to ensure timely flow of care for 6-7 patients simultaneously

Med-surg/tele Nurse

•	treat 12-15 patients each day
•	follow Doctor's orders

Nurse Supervisor- Floor nurse

•	Effective Case Manager organizing all disciplines providing services for client in home
•	Communicating with Physicians to establish patient specific parameters
•	Collaborating with other disciplines to ensure follow through of patient centered plans of care 
•	Ensuring timely and accurate submission of nursing documentation
•	Responsible for interdisciplinary management and receiving RN report for approximately 30 patient census
•	Ensuring nurse teaching and patient learning.  
•	Review OASIS and other Nursing documentation and ensuring medical records meet Medicare guidelines
•	Execute supervisory visits for LPN’s and Certified Nursing Aides
•	Represent Agency to all external personnel including at times of state and Medicare review 
 


Director of Nursing

•	Monitor caseloads and provide leadership for staff in peak periods
•	Monitor clinical record review, case coordination and oversight.
