Cory Mellon

● Evaluate, diagnose, and treat common primary care, urgent care, and mental health
conditions.
● Order and interpret diagnostic tests, X-rays, or Ultrasound to identify patient’s
condition.
● Make appropriate referrals to Emergency Departments or specialties as needed.


Family Nurse Practitioner

● Evaluate, diagnose, and treat common chronic and acute medical conditions via
telemedicine.
● Order, interpret, and evaluate diagnostic tests to identify and assess patient’s
condition.
● Performs annual physical assessments.

● Evaluate, diagnose, and treat COVID and STIs via telemedicine.
● Monitor and supervise telehealth appointments with medical residents.
● Make appropriate referrals for medical nutrition therapy, Medicare Home health
services, and Collaborative Mental Health care model.

● Perform complete medical histories and physical examinations as well as screening
tests for Diabetes, lipid panel, and Hepatitis.
● Identify gaps in members’ current medical treatment plan of care and make
appropriate referrals.
● Navigate members to appropriate Adult Protective Services, and mental health
services.

Nurse Practitioner

● Initiate Palliative consultation and make recommendations for patients to return to
Post-Acute Nursing facilities or maintain at home.
● Manage palliative symptoms, and prescribe treatment and medications for terminal
diseases.
● Visit patients at home to complete physical assessment, perform Face2Face
recertification, and evaluate hospice eligibility.
● Actively participate in Interdisciplinary team meetings and provide medical
guidance for care plans.

● Certified in Trauma Nursing Core Courses (TNCC) and Emergency
Communications Registered Nurse (ECRN).
● Proficient clinical skills: IVs, pushes, drips, 12-lead EKG, cardiac monitors, blood
transfusion, venous/arterial blood gases, and ventilator monitoring.
● Provide care in a non-judgmental, non-discriminatory manner to preserve patient’s
autonomy, dignity, and rights.
● Leadership and preceptor experience in training new-hired nurses.
